Getting Started with Your Credit Application

To begin, please complete the attached Business Credit Application. Every field must be filled in—if a question does not apply, enter a dash (–). Incomplete applications will be returned.

Each owner with 20% or more ownership must also complete an Individual Credit Application and a Personal Guaranty.

Before submitting, make sure you have:

• For the Applicant:

  • Business Credit Application

  • Voided check for EFT

  • Most recent Federal Income Tax Return

  • Financials for the last 24 months (income statement + balance sheet)

• For Each Owner:

  • Individual Credit Application

  • Signed Personal Guaranty

  • Most recent Federal Income Tax Return

  • (Optional) Last paystub for owner & spouse

Please ensure Social Security Numbers are correct. A credit report will be run on each owner and their spouse.
